You’ll need to complete a claims form – this can be downloaded from this site or you can call on 020 8662 8195 and we’ll pop a form in the post for you.
Part A of the form should be completed by you and Part B should be completed by either your GP or the hospital consultant. Any fee charged for the completion of the form has to be paid by you. Once you have completed then simply send the completed form back to us to the address on the form.
If we need to obtain further medical evidence we let you know who we have written to and will keep you regularly updated on the progress of your claim. The cost of any additional reports will be paid by us.
