Making a claim
  • You’ll need to complete a claims form – this can be downloaded from this site or you can call on 020 8662 8195 and we’ll pop a form in the post for you.

    Part A of the form should be completed by you and Part B should be completed by either your GP or the hospital consultant. Any fee charged for the completion of the form has to be paid by you. Once you have completed then simply send the completed form back to us to the address on the form.

    If we need to obtain further medical evidence we let you know who we have written to and will keep you regularly updated on the progress of your claim. The cost of any additional reports will be paid by us.

Claim Specific FAQs
    1. Is motorcycling covered?
      Yes, the Chartis Direct Personal Accident Plan does cover you should you be involved in an accident whilst riding a motorbike. However, all benefits payable under the Plan are halved for accidents whilst riding a motorbike.
       
    2. Are skiing accidents covered?
      Yes, the Chartis Direct Personal Accident Plan does cover you while skiing as long as not doing so in a professional or semi-professional basis.
       
    3. Is riding a jet ski covered?
      Yes, the Chartis Direct Personal Accident Plan does cover you whilst riding a jet ski.
       
    4. What is not covered?
      No benefit will be paid for a loss resulting from an accident:
      1. If the accident occurs in a country where a state of war exists (declared or not) if the accident was the direct consequence of the war.
      2. While you are flying unless you are a fare–paying passenger on a commercial flight.
      3. If you take a drug or drugs.
      4. If your injuries are intentionally self-inflicted.
      5. If you commit or attempt to commit suicide.
      6. While you are committing or attempting to commit a crime.
      7. If it results in fibromyalgia, myalgic encephalomyelitis [ME], chronic fatigue syndrome, post–traumatic stress disorder or any mental or nervous disorder.
      8. If it results from sickness or disease.
      9. If you reside outside the UK for more than 180 days.
            
    5. Do the rates go up as I get older?
      No, your rate will not increase as you get older.
         
    6. To whom are the benefits of the Plan paid?
      All the benefits are paid to the person who is injured. If the injured person is a child the benefit is paid to the policyholder if they are the parent of the child. If the injured person dies, the benefits are paid to the insured person’s legal representative or executor.
        
    7. When is my cover effective?
      Your cover will be effective immediately once we process your completed application.
        
    8. Is this life insurance?
      No, this is Personal Accident cover which covers you for accidents resulting in a permanent and temporary injury, hospital stays over 24hrs and accidental death as the result of an accident (as specified in the policy).

    We will acknowledge your complaint and keep you regularly informed about the progress of your complaint. For complaints relating to claims, it may take us a little longer to respond to you, especially if we need to consult with medical professionals, however we will let you know what information we are waiting for.

    We will do our best to resolve the complaint quickly and will issue a final response letter to you addressing the issues raised. If we are not able to resolve your complaint to your satisfaction you may be entitled to refer any disagreement to the Financial Ombudsman Service to review your case, without affecting your legal right to take action.
